Sara Voice + Text is the AI receptionist we are designing on top of the ComplianceOS HIPAA chassis. It is in active development today — no production deployments yet. What we are building

The capabilities below describe our day-one target. None of this is shipping today — we are sharing the design openly so founder-cohort practices can push back before we build the wrong thing.

Voice (planned)

Designed to pick up with your practice's name and tone, book and reschedule against your calendar, take intake details, and route urgent clinical questions to the on-call provider through your existing pager flow. Insurance capture is on the roadmap pending payer-data review.

SMS (planned)

Designed for two-way conversational texting from your existing business number — confirmations, reschedules, intake links, and post-visit follow-up — written to read like a front-desk text rather than a marketing blast. Carrier provisioning and 10DLC compliance are part of the build.

Email (planned)

Designed to read inbound mail, draft a reply in your practice's voice, and either send or queue for sign-off based on rules you set. The goal is for routine new-patient inquiries to convert into booked appointments without a staffer touching the keyboard.

For medical & dental practices

Designed to live inside your HIPAA chassis — not bolted on.

Most existing "AI receptionist" tools require a new BAA, export PHI to a third-party LLM, and ship their own audit trail you have to defend. Our intent is different: build Sara as a feature of ComplianceOS so it runs inside the same tenant where your audits, training, and incident logs already live. We are still designing the security architecture and have not yet completed a third-party review.

Design goal: your BAA covers it

If we hit the design, Sara ships as a feature of ComplianceOS rather than a separate vendor, so the BAA you sign for ComplianceOS extends to the receptionist. We will publish the final BAA language with the founder cohort before any practice goes live.

Design goal: PHI stays inside the chassis

Call recordings, transcripts, and message history are intended to live in your tenant rather than on a third-party voice or LLM provider's servers, with PHI redacted before any model call that crosses a boundary. The redaction approach is still being implemented and will be reviewed before launch.

Design goal: auditor-ready by default

Plan is to log every call, text, and email into the same evidence package your auditor already reviews — no separate bolt-on system to explain. Final evidence schema will be confirmed with the founder cohort.

These numbers are the founder-cohort target. Final pricing is set in writing with each cohort member before any charge. If we cannot deliver the scope below, we do not bill.

FAQ

Honest answers about an unfinished product.

Is there a working AI receptionist I can buy or try right now?

No. There is no working Sara Voice + Text product today. This page describes what we are designing and the founder cohort we are forming to help shape it. If anything on this page reads as if a product already exists, treat it as a design intent, not a current capability.

What does "founder seat" actually buy me?

A seat in the design conversation, early prototypes once they exist, and the first option to lock in founder pricing if and when we ship. It does not charge a card today, it does not commit you to buy, and it does not guarantee a ship date. If we never ship, no one pays.

How will it sync with my booking system once it ships?

The plan is OAuth integrations for Google Calendar and Microsoft 365, the ShopOS queue, and a small set of common PMS targets, with a structured email/SMS handoff fallback for older systems. The supported list is not finalized — we will confirm what your booking system needs during cohort design and tell you up front if we cannot support it.

When will it actually ship?

Our target is Q3 2026. That date may slip. We will be honest with the founder cohort about progress and timing rather than recommitting to a date we cannot hit. Founder-cohort participants only start paying after Sara is live in their office and they have signed off on the scope — not before.

Will it replace our receptionist or augment them?

Our design intent is to augment. The aim is to take routine work off the front desk — confirmations, reschedules, after-hours calls, lapsed-patient outreach — so staff can focus on the people in the room. Whether that fits your shop depends on your workflow, and we want to hear about it during the cohort.

What about HIPAA — will this be safe for a medical practice?

The architecture we are designing is intended to make Sara a feature of ComplianceOS so your existing BAA covers it, with PHI redacted before any model call that leaves the tenant boundary and recordings and transcripts stored in your tenant. We have not yet completed the implementation or a third-party security review, and we will not bring any practice live until that work is done and documented.

What happens when Sara does not know the answer?

The design plan is to allow per-workflow rules: hand off live to a staff member, take a callback request and queue it, or escalate to the on-call provider via your existing pager or phone tree. The intent is for Sara to never invent an answer to a clinical question and never book outside the rules you set. Cohort feedback will refine these defaults.

Can I hear Sara before I commit?

Not yet — there is nothing to demo today. As prototypes come online, we will share recorded examples with the founder cohort and, when a sandbox is ready, run live demos with cohort practices. Joining the cohort is how you get on the list for those.
